How to Register

If you live within 4.5 miles (as the crow flies) of our practice you are welcome to register with us. You can check your distance from our practice by clicking here.

If you live outside our practice area, we regret to inform that we are unable to register you with the practice; we invite you to check nidirect – Find a GP practice to see what facilities are closer to you, and better placed to meet your needs.

Which Registration form?         

HS200 for NORTHERN IRELAND & UK Residents ONLY. This form must be completed if you wish to register with our practice.You will also be required to supply Photographic ID & Proof of a current Northern Ireland address.   

HSC/R1 –  This form is used for individuals entering Northern Ireland coming from any any other country OUTSIDE the UK. You will also be required to supply Photographic ID & Proof of a current Northern Ireland address. Any person registering from outside the EU must submit a copy of their Visa or Work Permit. 

How do I get my forms?

You can visit the surgery to obtain forms, and complete it there and then, or take them away and complete and return at your earliest convenience. Please bring original copies of the supporting documentation required; this will be photocopied and returned to you.

You can also download the forms online & return to reception. Please ensure you complete the correct form (see above). Please also ensure you complete the ‘new patient questionnaire’ below and provide any ID required.

Entitlement to NHS services and first time registration (hscni.net)

New patient questionnaire

All new patients will also need to complete a new patient health questionnaire giving details of your important past medical history, medication you are on, allergies etc. We require proof of all medication you are already prescribed.

Additional Documents Required

Please see list on Page 8 of HSCR1 registration form for documents that are required for you to register, please take the time to closely examine this; the below examples are not exhaustive.

  • That you are lawfully in Northern Ireland (e.g. valid visa, EU or UK passport)
  • That you are residing in Northern Ireland (e.g. proof of address such as a utility bill, or tenancy agreement)
  • That you have a reason for being in Northern Ireland (eg. payslip, tax credit letter)

The above are required to assess entitlement to free NHS services in Northern Ireland.

We would ask that you pay very close attention to documentation required and answer all appropriate questions.  Failure to do so may delay your registration. 

If your application is refused

If you live over a certain distance away from a practice, or if it has closed its patient list, the practice may refuse your application. If this happens, you should contact the Business Services Organisation and ask them to put your name on a doctor’s list in your area.

Registering your baby

You need to fill in a GRO 4 Birth Registration Form. The General Registry Office will give you this when you register your baby’s birth. Bring this form to the GP’s surgery to register your baby as a patient. 

Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes. All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.

Text Consent

Our most common form of communication with our patients is via text messaging. We would ask that you please sign up to our text messaging service. This allows you to receive results, practice invitations & be kept up to date with relevant practice information via text message. Please complete our communication consent form.

Organ Donation

Most people would accept an organ if they needed one. Not surprising really. Yet only 49% of us have signed the NHS Organ Donor Register.

Last year 11 people died in Northern Ireland waiting for an organ transplant. By signing the register and talking to your family about your decision you could help save the lives of up to nine people.

Please be advised when signing documents to become an Organ Donor patients must be 18 or over. Parents or guardians signing on behalf of under 18’s is not acceptable.
